I think you guys are over-thinking this story. Handicapped?? I didn't get that at all, and I'm really not sure what you're talking about regarding the character interactions. What we're dealing with here is an unapologetically implausible tale, set in a universe with its own little rules, such as people being able to get away with poo and pee accidents without other people noticing, and friends behaving towards each other in a way that is slightly different from how they would in the real world.
So Courtney prefers to politely ignore Jody's discomfort, rather than offer to help. That would be rather mean of her in our universe, but in this one, perhaps it's just what friends do. In our universe, absolutely nobody in their right mind would be so shy as to avoid using a bathroom within earshot of her friends, even when incredibly desperate; in this one, perhaps it's not that uncommon.
I use this same trick in my own stories - I cheerfully describe incredibly implausible behaviour and decision-making, such as a woman wading through knee-deep poo to clean a mountain of poo off a toilet, just so she can poop in it. It's not realistic and is not trying to be. But it makes for a fun story.
