I rule A out immediately as being simply preposterous.
I think the salient point is that the two are attracted to each other. That gives him a powerful reason to watch her and, thus, it would take a powerful reason for him to stop doing so.
However the inability to converse (and, thus, lack of contact so far) pegs him in my book as afraid to offend her.
Consequently, given the facts as presented, I think B is most likely.
If it were C, he would have done the same thing the first time; since he did not, I interpret that as a realization that he may have offended her the first time by watching her in her moment of need when (he thinks) she did not want to eye-flirt.
I think to hypothesize C given these facts you almost have to assume that he didn't know she were going to the bathroom the first time and, hence, didn't realize the situation. But his continuing to follow her with his eyes on her exit the first time blows that theory.
