Underwear vs. diapers is a question of what her role age is...there is certainly no reason to take her out of diapers if she has become comfortable in them!
What role accessories have you tried besides diapers?How does she feel about sleeping with a pacifier in her mouth...or sucking her thumb?
How do you feel about spoon- or bottle-feeding her,or bathing her or dressing and undressing her?
