By accident do you mean staged roleplay or actual involuntary pooping?
I expect that only a couple who wanted her to mess her diapers would engage in such play,and apart from deliberate assistance from things like laxative suppositories it would take a lot of training before she could really mess without control.
Generally wetting diapers is far more common and messing is only for those who want to take things further...though of course real babies do do both and it's more faithful to AB role if the woman does too.
