Well,the most important thing is NOT TO STOP TRYING.You obviously have serious inhibitions to overcome but giving up has to be ruled out.
If you are at the point of a serious gotta-go urge perhaps he can hold you tight and talk you through it.And remember,the first wetting is the hardest...once you're over the hump it should get easier.
