Personally, I've had some modicum of success in authorizing orgasms as rewards for diaper use (especially messing). Granted, this is only a reward if such activity is otherwise restricted, but its the best idea I've tried, thus far.
Naturally, this does require some ability to trust that she's otherwise following a restriction on self-stimulation, and is probably best paired with a "photographic proof of diaper use" model (like that which was employed on Keptindypers.com).
