I would think of it from a different perspective. Gender confusion and age play confusion are a big part of it. And I wouldn't underestimate the power of human belief. If you truly believe in your heart of hearts that you happen to be a sweet little girl who am I to tell you otherwise just because your equipment is a bit different? The internet has added to this by turning it up to ten and ripping the knob off. I've asked myself the same questions and found that I am a sweet little boy when I play, but a very submissive, feminine little boy. And it took me a long, long time to figure all of that out. No wonder lots of men get lost along the way and anger the girls with the actual equipment.
