I rather think it might suggest something to you that the very sentence you wrote to express your thoughts in the most natural way flatly contradicts itself. Did the hypothetical "guy" in question "become" a girl or didn't "he"? You could fix your sentence to make your stance clear - eg, "If a guy has had the operation to give himself a neovagina, that doesn't mean he is a girl" - but wouldn't it sound a little stilted and cruel? No? Just me, maybe.
