By and large, it hasn't been...even when my abusive ex-GF was yelling "diaper pants" in public at the top of her lungs.
Of course, there are only two people in my life with any idea of what is going on...the one doesn't care and the other would just assume not know, so I continue to hide it unless there's a reason to talk about it. Only one person ever asked about why I crinkled, and I did not say.
But with a paper-like outer cover on my diaper, my pants have to fall down or the diaper has to leak in order to give away a clue. (I've been using super or super plus absorbency Depends protective underwear, am about to switch to something better from, probably, ABAIP). On the other hand, there ARE certain things I do where I do not want to wear a diaper...for example, canoeing (and swimming) in our local river, and probably not water rides at the local amusement park.
IMO, the trick to general acceptance is to wear them like you have to, not like you want to. The two people who know have very strong reasons to accept me, one is my GF and the other is a BDSM-oriented guy I work with. If more average people think you are getting off on wearing diapers or showing them, they won't accept you, in the same way they would reject someone who had intimate french kissing sessions in front of them while tickling their crotch. That much public sex is way outside their comfort zone.
