Some people simply have a fear of public restrooms. I have met people like this before, they will not use a public restroom no matter what. Usually it has to do with them thinking that the toilets arent clean. In all cases it is women, never met a man afraid to use public toilet. Then again, a man doesnt have to make physical contact with the seat.
