It's strange but true, most people won't say anything, even if they notice. They assume you have had "an unfortunate accident." The idea that you might have done it on purpose for a kinky sexual thrill never even crosses their mind.
Only once have I ever been confronted by someone who seemed to know I had done it on purpose. It was a guy who was standing behind me in a convenience store and started saying, very loudly, "I think there's a big baby in here who needs to change her diapers!"
That wasn't fun, actually kind of creepy and scary. Usually, people pretend they don't see you, but then point and whisper behind your back. If that sort of thing bothers you, maybe you shouldn't try it. Personally I sort of get off on it.
