It's different in every case I would think, Larry.
Personally I think I'm quite ugly, but I *do* get a buzz when someone watches a panty-pooping video I've made and writes to me and says "wow - you're gorgeous, that was awesome" ... and I think a big part of the reason I try and do websites etc. stems from that.
Also, because I'm very into "fear-wetting/messing", some of the thoughts going through my mind when I poo in my pants are based around being humiliated or terrified.
I think that I'm a happy person most of the time though!! I'd never describe myself by saying "low-self-esteem", but I've been hurt a lot (and recently).
One thing you have to keep in mind is that people are into fetishes for tons of different reasons, so you can never say for certain what state of mind they are all in at any given time...
