Personally, I would just ask HER all of those questions. Any of us could advise you on what we think is best, but NONE of us could tell you the first thing about what your girlfriend will like or won't like.
If you're uncomfortable with sitting her down and having a lengthy discussion about it all in one go (sort of a "What about... this? Or this? Hey, and what about this?" type thing) then take it VERY slowly. Throwing elements she doesn't like or isn't comfortable with at her all at once might turn her off to the whole thing.
In other words, either sit her down and discuss ALL the aspects of it before you play (unsexy, I know, but truly the healthiest way to proceed) or go SLOWLY and make suggestions along the way, feeling her out as best you can. Every woman is different... and you need to find out what makes yours tick.
